simple invoice script

I basically need a simple invoice php script.
The way it should work is I would log into it, protected by a username and password that is php based, not htaccess based.
I simply enter the item name, description, and price. Then click on "ADD".
It should create a professional looking "order" page. The order page will have the details I entered above and below that, the fields for the order form.
Things like:
Name, Address, Credit card number, expiration, CVV code, and so on.
